Learning Disabilities 

We offer an annual assessment for all patients with Learning Disabilities. At this appointment it gives the patient the opportunity to ask questions about your health, and your GP can refer to a specialist if it is needed. During the appointment a general health check will be done to makesure you are doing well with your health so you get the best treatment. Our nurses can give you information that is easy to read so this helps you to understand what tests may need to be done. If you also have a Carer there is also information on hand to support your Carer too. After the appointment an action plan will be given to you which you can follow to help with your health. 

Please ensure that you attend your appointment for your Health check when you recieve an invite.
